For applications

(General) physicians can register their patients through Zorgdomein.

Individuals and other referrers who do not have access to Zorgdomein can send a perspective plan or a referral letter from the general practitioner to aanmelden@homebasezorg.nl. You will be contacted within 14 business days at the latest to discuss your application further or to schedule an intake interview.
