For applications

(General) physicians can register their patients through Zorgdomein.

PLEASE NOTE: due to the vacation periods and limited availability of therapists, WE DON'T TAKE NEW APPLICATIONS FOR ADULT Mental Health until 25 AUGUST. YOUTH MAY STILL APPLY.

Individuals and other referring physicians who do not have access to Zorgdomein can send a message to aanmelden@homebasezorg.nl that you would like to enroll yourself or someone else. You will be sent an application form via secure email. Please return the completed form along with a referral letter and any other information to finalize your registration. You will be contacted within 14 business days at the latest to discuss your application further.
